For many older adults, living with chronic health conditions is part of daily life. In fact, national studies indicate that the majority of residents in assisted living communities manage more than one ongoing condition.

According to the CDC, the 10 most frequently observed chronic conditions among residential care community residents include:

•  High blood pressure (58%)

•  Alzheimer’s disease or other dementias (44%)

•  Heart disease (33%)

•  Depression (26%)

•  Arthritis (18%)

•  Chronic obstructive pulmonary disease (COPD) (16%)

•  Diabetes (16%)

•  Osteoporosis (12%)

•  Stroke (7%)

•  Cancer (6%)

More than half of assisted living residents (55%) have been diagnosed with two or three of these conditions, underscoring the importance of coordinated, compassionate care.

 How We Support Residents

At Traditions at North Bend, in North Bend, Ohio, our nursing team partners closely with each resident’s primary care physician and disease specialists to provide personalized support, through individualized service plans and frequent health evaluations, we help residents manage their conditions in a home-like setting that emphasizes comfort and dignity.

Our goals are simple:

•  Promote overall wellness

•  Support each resident’s optimum level of health

•  Coordinate care with medical providers

•  Reduce unnecessary hospitalizations

This approach is especially important for conditions such as congestive heart failure (CHF), COPD, and diabetes mellitus (DM), three of the most common chronic illnesses linked to higher rates of hospitalizations and 30-day readmissions. By staying proactive and collaborative, we help residents live fuller, healthier lives.

Our Commitment to Improving Quality of Life for Our Residents

Managing chronic conditions is about more than medicine; it’s about ensuring residents feel supported, understood, and cared for. At Traditions at North Bend, we are proud to walk alongside our residents and their families, offering compassion, community, and peace of mind.
